This is a very informative article from Tom Jesson, Physiotherapist and author of “The Clinician’s guide to Sciatica’ “Understanding disc herniation: a historical perspective’.

In summary Tom says:-

‘ Our knowledge that disc herniations cause radicular pain, not only through compression but also through chemical irritation, allows us to look beyond surgery as the only treatment option and begin to explain otherwise confounding presentations such as people who have radicular pain, but with little or no apparent herniation. Finally, the finding that herniations occur not only because of injury but also because of age, microstructure degeneration and genetic bad luck allowed us to move beyond simplistic “protect your spine” advice and educate our patients that load, within reason, is good for discs.’
